GENERAL PURPOSE:Participate in Case Management and Discharge Planning activities.ESSENTIAL FUNCTIONS:Evaluate patient care outcomes, medication administration and care management. May be required to give direct patient care, such as bathing, dressing, feeding, and assisting with examinations and treatments of hospital or LTC patients. Ability to lift, walk, carry and stand. Must be able to be on administrative on-call status rotation as needed for nursing administration. DUTIES AND RESPONSIBILITIES: 1.Facilitate, in collaboration with the Clinical Managers, in-service education for clinical staff. 2.Keep Case Management Manager informed of departmental activities, needs and problems. 3.Maintain close liaison with other hospital departments to assure coordination, standardization and continuity of patient care. 4.Plan effective discharges and transitions of care. 5.Work with patients and families to achieve mutually acceptable discharge plans. 6.Provide resource information to patients and families regarding care options and community support services. 7.Perform community outreach education as needed. 8.Work with patients, families and physicians to promote effective use of resources. 9.Participate in hospital-wide Quality Improvement activities and is a resource to Department Managers.
EDUCATION AND EXPERIENCE REQUIRED: 1.Graduate from an accredited School of Nursing. 2.Five years of clinical experience preferred. 3.Current Nevada Nursing Licensure. Preferable professional organization membership on a state or national level pertaining to Case Management.
