Providence is calling a per diem, varied shift Registered Nurse Case Manager to Providence Little Company of Mary Medical Center in San Pedro, CA.
In this position you will:
Focuses on the integrated function of utilization review and discharge planning of the hospitalized patient.
Performs discharge planning byassessing the patient's needs; identifies the resources to meet those needs andcoordinates a smooth transition from one level of care to another.
Responds to patients and families withknowledge of age specific, infant, pediatric, adolescent, young adult, middleadult and older adult needs based on scope of service of unit/department whereassigned.
Required qualifications for this position include:
Current California Registered Nurse (RN) license
At least two years of acute hospital experience
Strong knowledge base or experience in specialty area assigned
Preferred qualifications for this position include:
Case Management experience
Bachelor's degree in Nursing
Case Management experience
Critical Care of Emergency department experience
