The Discharge Planner works closely with the Clinical Department and the Utilization Review Department in ensuring that a smooth transition occurs with all inpatient discharges from the facility. They are responsible in communicating with the patients and providing options available through their insurance plans and discussing with patients and family members discharge options available to them. They assist the patients and family members in finalizing the discharge plans. Ability to read and interpret hospital documents and insurance plans. Has the ability to verbalize orally and in writing with patients, family members, clinical staff and referral sources. Ability to analyze medical records documentation and can communicate a clinical formulation. Ability to problem solve in a variety of situations.
Bachelor's Degree in Behavioral Health preferably in Social Work. Also prefer a minimum of 1-year experience in case management or discharge planning from an inpatient mental health/addictions facility..
