The Transition Coordinator supports adolescents and young adults with complex medical conditions and/or developmental disabilities as they prepare for transition to adult healthcare, life and services. The Transition Coordinator also serves as a resource on transition and the needs of transition-age youth both internally and externally.
Develop and maintain a directory of providers, programs, supports and resources for young adults with complex medical conditions and/or developmental disabilities
Participate in clinical appointments to provide customized information to adolescent and young adult patients and their families
Support current and former transitioning patients and families Participate in outreach and education efforts related to transition, internally and externally, including coordination of the resident and medical student transition elective experiences
Participate in committees, workgroups, conferences and quality improvement initiatives as needed to advocate for transition-age youth, internally and externally
Participate in data tracking, research and grant writing
Participate in departmental meetings and committees as assigned
Utilize effective interpersonal and organizational skills Document in the medical record in accordance with departmental guidelines and mandates Comply with all policies and procedures of the department and hospital
Master’s degree in Social Work from a program accredited by the Council on Social Work Education (CSWE), or Master’s degree or similar educational background in a relevant field
At least 2 to 5 years professional experience preferred.
