The Integrated Behavioral Health Program is a critical part of Marin Community Clinics’ service model. We serve patients with behavioral health concerns of all ages through an integrated behavioral health & team-based model of care.
We are seeking a dynamic behavioral health leader to enhance MCC’s ability to respond “in the moment” to patients with acute psychosocial needs. The Triage Behavioral Health Provider (T-BHP) works as a member of the primary care team to provide urgent consults, biopsychosocial assessments, crisis intervention, education, referrals, and consultation (“warm handoffs”) for patients presenting to MCC with acute behavioral health or psychosocial concerns. The T-BHP has the opportunity to work with patients across the lifespan and with a wide variety of clinical presentations. The T-BHP may provide consultation in-person or (via tele-health across MCC’s 5 clinic or dental sites). The T-BHP is welcomed and supported by a dynamic behavioral health department and primary care team.
Work Schedule: Monday through Friday, 8:00am to 5:00pm.

Provides “on-demand” triage services for patients presenting to MCC with acute psychosocial needs via tele-health or in-person at their “home clinic”.
Provides psychosocial assessment, brief behavioral interventions education & referral to adult and pediatric primary care patients with mental health, substance abuse, and other psychosocial concerns;
Ability to assign a diagnosis based on DSM criteria and delineate appropriate treatment plan;
Provides crisis intervention, including assessing for suicidal risk, IPV, and other high risk clinical scenarios;
Provides consultation to medical providers regarding the behavioral health components of a patient’s presentation, and develops collaborative plans to address symptoms and enhance functioning;
Works with patients in a culturally competent manner in their preferred language (primarily English and Spanish);
Uses the “warm hand off” method (immediate brief assessment and intervention) for assisting patients with identified behavioral health issues at the time of their medical visit;
Engages in ongoing learning about new medications and treatments commonly used for mental disorders, substance abuse, and general health problems.
Able to maintain a whole-person, strengths-based approach while working within a fast-paced medical clinic;
Uses the Clinic’s electronic health record (EHR) to document clinical work, produce billing, collect service data, and maintain a schedule, as well as to communicate information to other clinic providers.
Assists in developing and maintaining procedures and protocols to ensure the Behavioral Health program’s relevance and effectiveness for patients and clinic staff.
Attends medical provider meetings, all staff meetings, and behavioral health team meetings as scheduled.
Provides frank and considered opinions regarding quality improvement measures.
Able to engage and treat patients effectively in high-volume, fast-paced clinic setting and demonstrates ability to model good self-care;
Provides Behavioral Health care to satellite clinics.
May be asked to work variable schedule during extended-hour clinics (e.g. 10am-7pm on one or more days).
Other duties may be assigned.
Licensed Clinical Social Worker (LCSW) or Licensed Psychologist (PsyD/PhD)
Must have the ability to effectively present information and respond to questions from clients, customers, and the general public in Spanish.
Must be able to conduct clinical assessment interviews and intervention with patients in Spanish.
Experience with tele-health desired;
Experience with & commitment to motivational interviewing;
Experience using evidence-based intervention in brief treatment format;
Commitment to cultural humility required;
Demonstrated ability to work effectively on a team, show initiative, flexibility and enhance a professional culture of trust and good will;
Experience in primary care environment preferred.
Previous clinical experience should include DSM V assessment and diagnosis and crisis intervention.
Knowledge of state regulations and standards related to client treatment, patient rights, and client/patient confidentiality required. Familiarity with psychopharmacology is helpful. Demonstrated ability to work effectively on a team, preferably in a primary care environment. Ability to see 4 patients per 4 hour shift. CPR Training Current. Valid Driver’s License. Must be computer literate and knowledgeable in Microsoft Office software programs, and how to conduct searches on the Internet.
