The San Francisco VA is seeking a full-time, permanent Social Worker (Caregiver Support Coordinator). The Caregiver Support Coordinator (CSC) is a Social Work Program Coordinator whose primary responsibilities are to process and manage Veteran and Caregiver applications to the Program of Comprehensive Assistance to Family Caregivers (PCAFC). This includes completion of psychosocial assessments of Veteran and Caregiver applicants. This may include Wellness Visits, held virtually or in the home; short term case management; linkage and referral; Caregiver support; clinical evidence-based services and interventions; program development; caregiver, Veteran and staff education on caregiver issue. May participate in community outreach; continuous quality improvement activities; and evaluation/consultation. The CSC is administratively responsible for providing clinical and other daily operations activities of the program. May participate in development of policies and procedures for program operation, preparation of statistics for program, facility, VISN, and national use and in program evaluation . Reports to the Caregiver Support Program Manager. The CSC, must be a Licensed Clinical Social Worker, license by a state to practice at the advanced practice level and will function in support of caregivers, Veterans and family members, the medical center and in community settings. The CSC provides consultation and guidance to colleagues, role model effective social work practice skills, teaches or provides orientation to less experienced social workers, develop innovations in practice interventions, and may provide clinical supervision for social work licensure or certification. The CSC in this position requires advanced knowledge and understanding of the services and benefits available for Caregivers from both federal, state and private sector resources. The CSC independently manages a complex caseload that includes disabling mental, physical and geriatric-specific conditions. The CSC interfaces with multiple VA healthcare services and with community agencies.
