Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care including documentation. Assesses, plans, implements and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management during and after discharge, and supporting factors that impact customer satisfaction. The Registered Nurse (RN) selected for this position is accountable to the Associate Nurse Executive for Primary Care Clinics and works under the direction of the Nurse Manager and within identified Specialty outpatient clinic teams. Responsible for the observation, treatment and care of the Veteran within the scope of Registered Nurse practice. Assists in the overall clinic management, functions and operations, facilitation of patient flow through the check in and check out process, COVID swab testing, COVID screening , COVID/FLU vaccine clinics and provides patient education and/or instructions based on identified needs, education level and ability to learn and validates understanding with appropriate documentation in the medical record. Works with multiple specialty clinics. Participates in job/systems redesign activities and other activities that enhance patient care and clinic operations. Maintains patient's privacy and confidentiality. Assists with care coordination, management and facilitates coordination of all services for the patient in the primary care clinics works with the patient, significant other and health care team to establish patient centered care. Demonstrates the knowledge and skill sufficient to assist with procedures and use of reusable medical equipment as permitted by approved local facility policies and procedures. Observation and documentation will include clinical reminders and the reporting of any noted change in the patient's condition to the Physician. Provides leadership in interpreting, supporting and implementing Medical Center and Primary care philosophy, policies, procedures, standards, guidelines and quality improvement processes. Serves as consultant in clinical practice. Communicates and works effectively with others. Initiates and leads groups of personnel and interdisciplinary groups. The qualified RN demonstrates a solid foundation in nursing practice using the nursing process (assessment, nursing diagnosis, planning, implementation and evaluation).
