The Diagnostic Cancer Patient Navigator offers individualized assistance to patients, families and caregivers throughout the cancer care continuum. The navigator works to overcome health care system barriers; facilitates timely access to medical and psychosocial care; and offers education to assure patients and families have the resources, information, knowledge and support they need through all phases of diagnosis. This requires coordinating referrals, identifying and mobilizing resources, follow-up, and communication with members of the health care team. The Diagnostic Center Patient Navigator functions as a member of the health care team working towards optimum patient outcomes. The navigator duties include, but not limited to, participating in Oncology Committee to ensure compliance and maintenance of standards. The Navigator also helps coordinate Tumor Boards and Breast Conferences to include case preparation. Data collection and follow-up with physicians will also be required
DUTIES AND RESPONSIBILITIES:
Works closely with the mammography QA specialist, the imaging staff, physicians and surgeons to provide patient education and navigation.
Assist in breast biopsy procedures.
Assist patients in understanding their diagnosis, treatment options, and the resources available.
Must be able to work in a self-directed environment, with ability to work with and lead teams.
Serve as an essential link between patients and all other care providers.
Help coordinate multidisciplinary planning conferences.
Develop patient education and tools.
Ability to implement professional and community based education programs.
Work in cooperation with Cancer registry staff and others to collect data, track outcomes and support strategic planning processes.
Responsible for outreach efforts to establish and maintain positive working relationships with key physicians, office staff, diagnostic staff, social services and oncology staff.
Assist in program development and possess ability to implement policies and procedures.
Participates in patient care conference.
Demonstrates the ability to deal with pressure to meet deadlines, to be accurate, and to handle constantly changing situations.
Demonstrates the ability to deal with a variety of people, deal with stressful situations, and handle conflict.
Performs other duties as assigned.
Adheres to dress code.
Completes annual educational requirements.
Maintains regulatory requirements.
Wears identification while on duty.
Maintains confidentiality at all times.
Attends department staff meetings as required within the department.
Reports to work on time and as scheduled; completes work in designated time.
Represents the organization in a positive and professional manner.
Actively participates in performance improvement and continuous quality improvement (CQI) activities.
Coordinates efforts in meeting regulatory compliance, federal, state and local regulations and standards

Complies with Health and Safety Standards and Guidelines.
Current Montana RN license
Current BLS Certification.
Requires excellent leadership, organizational, written and verbal communication and interpersonal skills.
Three or more years of Oncology, Radiology or other health related field experience preferred.
