The VA Ann Arbor Healthcare System is seeking a Registered Nurse - Lung Cancer Screening Navigator to be a member of the Oncology Team in Medicine Service. This is a full time position. The Cancer Screening and Cancer Care Navigation Team (CSCNT) at the VA Ann Arbor Healthcare System (VAAAHCS) at the Stuart J. Rahr Precision Oncology Program of Excellence works collaboratively with the local and network VISN10, under the direction of the Precision Oncology Program Manager to assure that high risk patients are screened for lung cancer and have timely access to cancer care and needed psychosocial support. The CSCNT RN identifies Veterans for lung cancer screening and eliminates barriers to care and coordinates cancer care across the continuum and through transitions in care. The RN Care Manager in Oncology is assigned administrative and leadership responsibilities for the Oncology Clinic along with the designated clinic Physician, NP and Specialty Nurse Manager. The RN Care Manager provides direct patient care, coordinates care for patients followed in Oncology clinics, and performs administrative functions that ensure continuity of care and appropriate access to care. The incumbent will be responsible for individual and/or disease based care management and overall coordination of care for Oncology patients. The nurse must demonstrate leadership in delivering and improving holistic care to the adult and geriatric population through collaborative strategies and in compliance with either the Michigan or Ohio Nurse Practice Acts, in addition to Medical Center and Departmental standards. The incumbent provides direct nursing care to individual and groups of patients under the guidance and direction of the Nurse Manager (and Assistant Nurse Manager) and assists with the direction and supervision of other assigned clinical and/or clerical staff. He/she works closely with the NP, Administrative Assistant, Oncology Chief, Attending Providers and rotating Resident staff to ensure the timely delivery of quality care for our customers. Incumbent sets up, uses, reprocesses and maintains all unit/department-specific equipment that is designed by the manufacturer to be reused for multiple patients. All steps and activities for steps, reprocessing and maintenance will be completed in accordance with steps outlined in the equipment Standard Operating Procedure (SOP). He/she is responsible for the creation and review of Oncology Standard Operating Procedures and Competencies for all Reusable Medical Equipment. Duties of the CSCNT RN include but are not limited to: Identifying high risk Veterans for lung cancer using evidence-based standards Screening these veterans using evidence-based screening tools Acts upon cases of veterans with positive screening by utilizing an evidence-based methodology, which includes coordinating imaging, connecting with specialty services and/or clinical trials Working effectively with all levels of staff, promoting collaborative efforts, acting as a liaison between the patients and caregivers and the providers, and facilitating communication among the multidisciplinary care team to prevent fragmented or delayed care Participating in quantitative and qualitative data collection efforts as needed to support program evaluation and quality improvements The nurse is organizationally aligned with the Oncology Service. The nurse reports directly to the Specialty Clinic Nurse Manager for day to day clinical responsibilities and a annual clinical competence review is conducted by a designated nurse peer, nurse educator, or nurse supervisor.
