You Will Be Responsible For:
Reviews and maintains the resident matrix/roster daily. Updates pertinent information and changes after reviewing with the interdisciplinary team in daily CQI (Continuous Quality Improvement) meeting. Discusses changes with the MDS Coordinator and RN Director of Nursing.
Participate with staff in the observation of residents and the care planning process.
Offers input and suggestions to the Director of Nursing on how to deliver the best quality care for the residents.
Monitors staffing patterns including excessive overtime and minimum staffing.
Monitor resident infections and antibiotic use data and communicate to the appropriate individual for each episode of infection involving a resident on the designated form.
Provides supervision and structure for residents on the assigned units to determine that residents are provided assistance with meals to maintain nutritional status needs. Insures that residents participating in mealtime restorative programs are monitored and certified nursing assistants maintain accurate records of all resident oral intakes.
Coordinates the monthly drug regiment reviews for the unit with the Consultant Pharmacist. Advise Director of Nursing on the medication pass audits that require scheduling. Inspect the medication room and med carts to insure they are orderly, clean and sanitary as well as properly locked/secured.
Ensures a well-organized unit by following the facility policies to insure chain of command is followed and that each level of nursing personnel assigned to the unit fulfills their job description and renders quality care. Makes recommendations to the Director of Nursing when organization is lacking or a need for modification or improvement is identified.
Monitors staff to determine the unit is maintained in a clean, orderly and functional manner.
Nurse’s stations, medication carts and medication room are clean and secured.
Supplies are available to provide resident care and accounted for as well as utilized to prevent loss and/or waste. Utility rooms and storage areas are cleaned at least once per shift.
Attend Continuous Quality Improvement (CQI), department head meetings, and supervisory nursing meetings to insure the facility is progressing as one entity and following one set of quality standards. Is responsible for conducting unit meetings with staff to insure communication is complete. Works in a cooperative manner with other managers to share and provide a team spirit throughout the facility.
Responsible for making recommendations and improvement in departmental working systems and procedures by means of participating in the supervisory committees present within the facility.
Monitors staff in all aspects of job performance and policy requirements by means of Quality Assessment and Assurance audits. Audits consist of assigned unit review to insure standards and regulations are being followed.
Responsible for assisting the Director of Nursing in the investigating, documenting and reporting of any incidents. Route all original incident/ accident reports to the Director of Nursing within 24 to 48 hours.
Monitor all orientation documentation for completeness and return to Staff Developer or Director of Nursing.
Conducts ongoing rounds to determine that resident safety devices are appropriate and used as the resident’s plan of care indicates.
Reports resident grievances and family concerns to the Director of Nursing or designee.
Monitoring of 24-hour report, new orders and verification of census.
Perform clinical assessments as requested by licensed staff.
Uses prudent judgment and assists the Director of Nursing or on call supervisor in the decision of transferring residents to the emergency room or higher level of acuity, in accordance with the physician’s order. Ensure all pertinent document completed in EMR with transfer to higher level of care.
Assist in the review of the charts of resident’s that have been readmitted, newly admitted, or demonstrating a significant change.
Check charts on new admissions within 24 to 48 hours for completion. Complete new admission checklist and turn in to DON.
Monitors weight variances and request weights as needed. Can be pulled from Vision
Monitoring of pressure ulcer program, provide weekly reports to Director of Nurses as requested. Follow up of monthly consult reports to include, pharmacy, medical records, nutritional support, quality monitors, etc.
Update on a daily basis the Weekly Roster Matrix/ Census and Condition Reports, and submit to Director of Nursing as requested, at a minimum weekly.
Monitor for completion of required daily, weekly and monthly nursing documentation.
Resident TB/PPD, flu and pneumococcal vaccine monitoring and surveillance.
Monitoring of behavior monitoring flowsheets and AIMS testing for completion.
Completes daily care compliance rounds. Routes information requiring action to the appropriate individual(s), and the Director of Nurses.
Follow up on diagnostic test. Routes abnormal lab values to the appropriate parties for action when required.
Attend care plan meeting for the unit and audit/update the Nurse Aide Data Sheets audits the certified nursing assistant care plan as required.
Review staffing coverage and adjusts assignments to provide adequate coverage on each unit as assigned. Assist with nursing staff recruitment as assigned. Provide supervision of staff to assist in evaluation and identification of training needs for staff to maintain competency as required. Participate in employee evaluations as scheduled.
Participates in supervisor on call program as required.
Assist as requested by the Director of Nursing in developing a plan of correction for cited deficiencies following state and federal surveys. Monitor and audit as assigned to determine plan of correction is accomplished and compliance is present.
Audit wound care report and dressings to determine that care is being delivered according to the residents’ plan of care.
Audit resident’s restorative nursing program to determine the residents’ plan of care is being followed.
Report resident, family and staff grievances to the Director of Nursing and/or Social Services for action. Assist with resolution as directed.
Maintain compliance with Quality Assurance criteria and participate as requested by the Director of Nursing.
Be visible and supportive to staff and needs.
Monitor Resident’s Rights compliance.
Monitor resident positioning, restraint and releases.
Notify Director of Nursing with any changes in resident’s condition.
Responsible for overseeing the monthly physician order sheets are replaced and compliance is maintained per Adventist Care policies. Completed by medical records.
Ensure Physician orders, telephone orders and all transfer information is properly implemented on a timely basis and signed by the physician as require. Review and implement all new orders. Medical Records follows the signing of orders in Vision.
Monitor to ensure primary diagnosis is consistent with Medicare criteria for Medicare residents and that the documentation reflects the required criteria is being provided by the facility.
Attend weekly Medicare meeting with Restorative/Rehab and other staff to monitor Medicare resident’s status and documentations of need to continue with Medicare Coverage.
Review MDS assessments for the unit with the MDS Coordinator for completion on a timely according to MDS/PCP schedule. Update and ensure care plans for resident in Vision on the unit are complete for all care needed by assessments.
Check unit supplies to ensure amount is consistent with resident/staff needs and that the supplies used on the unit are accounted for.
Attend and participate as requested by the DON department meetings. Attend the Department Head meetings as scheduled.
Participate in disaster and fire drills when held and as assigned.
Conduct record audits on a regular basis. Special QA audits to be included in audit process as assigned by the DON.
Participate in the facility policy and procedure manual review and updating as assigned by the DON.
Ensure residents personal inventory records are updated at least annually and maintained as current.
Behaves in a professional manner. Behavior reflects the code of conduct expected by the Adventist Health Care System and our mission statement.
Open and complete Nursing Management Task List Process daily.
Monitor C.N.A document widget throughout the day to ensure proper ADL documentation is being completed.
Monitor Clinical Smartboard to ensure all medications are being administered on the unit in a timely manner.
Monitor the To-Do’s list to ensure any alerts from the touchscreen are handled appropriately and all open nursing processes are completed.
What You Will Need:
Must be 18 years of age or older
Current Basic Life Support (CPR) certification
Current Licensed Practical Nurse license in the state hired
Meet the state specific educational requirements for the nursing supervisory position.
Clinical supervision of Licensed Practical Nurses and Certified Nursing Assistants
Graduate of an accredited School of Nursing
Proficient in the use of a computer
Typing of 40 wpm
The Unit Manager participates in the direct care and services provide to the residents. The Unit Manager agrees to adhere to the principal duties and job requirements outlined within this policy. It is the Unit Manager’s responsibility to follow the policies and procedures of this organization. Adherence to the Federal and State statutes is mandatory. The professional nurse and the licensed practical nurse shall be responsible and accountable for making decisions that are based upon the individual’s educational preparation and experience in nursing. In addition to following the duties set forth in this policy, the nurse shall follow the directives of the State Board of Nursing in the state in which he/or she practices.
