The Specialty Care Clinical Pharmacy Specialist (CPS) in the ambulatory care setting must be a licensed pharmacist, preferably with a Pharm.D. or equivalent experience. The incumbent will be responsible for the strategic initiative SGLT2i to prevent cardiovascular disease and dialysis in patients with chronic kidney disease (CKD). Additional specialty care workload may be assigned based on the needs of the facility.
