The Clinical Pharmacist is responsible for performing medication reconciliations post-discharge and improving transitions of care. In addition, the clinical pharmacist will perform office visits with patients according to a collaborative practice. The Clinical Pharmacist will be responsible for managing and overseeing therapy for patient for a variety of chronic conditions; including but not limited to diabetes, hypertension, hyperlipidemia, COPD, asthma, insomnia, obesity, smoking cessation, anticoagulation, and chronic pain.
Primary Responsibilities:
- Manage and oversee therapy for a variety of chronic conditions; including but not limited to diabetes, hypertension, hyperlipidemia, COPD, asthma, insomnia, obesity, smoking cessation, anticoagulation, and chronic pain as part of a collaborative practice. Patients are referred to the clinical pharmacy by the physician or APC for long term chronic care management, which may occur as in-clinic, virtual or telephone encounters
- Provide medication reconciliation post-discharge and ensure appropriate guideline-based therapy, management of medication interactions, and appropriate transitions of care
- Utilize prescription and medical claim data to generate clinical recommendations according to Medication Therapy Management and Retrospective Drug Utilization Review program protocols
- Ensure performance goals are met for Stars/HEDIS via Portal utilization to provide clinical recommendations pertaining to, but not limited to, gaps in care, compliance and adherence
- Utilize client formulary information to guide appropriate medication recommendations, therapeutic substitution, and generic substitution
- Keep current with new and emerging clinical trends and present this information during monthly provider meetings
- Participate in departmental meetings to improve clinical programs and enhance processes
- Share clinical information and department procedure protocols during client site visits
- Assist with user acceptance testing for new clinical programs and system enhancements

Required Qualifications:
- Undergraduate degree or equivalent experience
- Current and unrestricted pharmacist licensure in Arizona
- 3+ years of experience working as a clinical pharmacist
- Experience conducting research and analysis
- Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements.
