The successful Immunocompromised Host Infectious Diseases (ICHID) Clinical Pharmacist Practitioner (CPP) will manage immunocompromised patients (e.g. oncology, solid organ and bone marrow transplant, and ventricular assist device (VAD) treated with oral and intravenous anti-infective therapies in the ambulatory care setti
